An OSHA visit can be caused from several different initiated events.
1) Targeted Inspections: Your specific industry may be the OSHA focus of the quarter.

An unhappy employee reports upon a specific hazard they feel has not been properly addressed. Whether real of imagined OSHA must investigate the issue.
